Course Curriculum

Population health management (PHM) is a framework that aims to improve a population’s health by focusing on the health needs of individuals. Effective PHM is crucial in the health care industry because it can reduce costs, improve quality, and enhance patient outcomes. Additionally, it can help reduce health disparities and achieve equity in health outcomes. In this session, we will discuss the importance of effective population health management in health care and its relationship with equity in health outcomes.


Continuing Education Credits 

In support of improving patient care, Huron is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

This activity was planned by, and for, the healthcare team, and learners will receive Interprofessional Continuing Education (IPCE) credits for learning and change.
